Instructions

Advance review of these instructions and necessary materials for your application is highly recommended, and it is the applicant’s responsibility to utilize these most up to date guidelines for the current application cycle. The Paramedic Department will only look at applications after the posted deadline to ensure fairness in the application process. If anything on your application (including attachments) is not provided, does not show your name, or is inaccessible to us, then we cannot consider your application complete, and it will not be reviewed. Should you need to make any changes or corrections to your application, you may submit a revision(Including all required attachments) any time before the deadline.

Student Eligibility

To be eligible to apply for the CCC Paramedic Program, candidates must meet the following criteria:

• Be currently certified as an EMT in the state of Arizona, or NREMT certified and able to obtain Arizona EMT certification upon being accepted into the program.
• Must be at least 18 years of age
• Have a current American Heart Association Basic Life Support for Healthcare Providers (CPR) card. (CPR certification must be maintained current throughout the paramedic training)
• Have a valid state driver’s license
• Have a high school diploma or GED
• Submit a complete program application and all supporting documents
• Show a positive appearance, motivation and attitude (in the clinical and field internship segments, the student will be working closely with patients, nurses, physicians and paramedics and must maintain a high degree of professionalism)
• Score a minimum of 65% or higher on the written Entrance Exam and Practical assessment
• Successfully complete an interview with the advisory board on the date and time provided
• Show verification of TB skin test and negative reading dated within 6 months of the start of the program
• Provide proof of vaccination in accordance with current Northern Arizona Healthcare requirements (To be supplied on first day of class. See list of required vaccinations below)
• Be able to read at a college level
• Be able to adapt to stressful situations

Required Documents

Please be prepared to upload the following documents with your application:

• High school diploma/GED or equivalent
• College Transcripts
• Current BLS HCP Card
• NREMT Certification (optional)
• Arizona EMT Certification
• Driver’s License

When uploading attachments, please ensure the following:

• Legal name is displayed that is consistent with the name that you are using on the application.
• If a different name is displayed on any of your documents, then you will need to provide proof of a name change, either through a marriage certificate or court order.
• Documents for upload must be in PDF format.

Application Review, Entrance Exams & Advisory Committee Interview

Once you submit your application to the paramedic program, it will be placed on file. EMS Education personnel will reach out with instructions for registering for your Entrance Exam. Your interview will be scheduled upon completion of the exam.

Required Vaccinations

The following vaccinations are required to participate in the Paramedic Academy. Documentation of all vaccinations will be submitted in accordance with Northern Arizona Healthcare policy. Be prepared to upload proof of all vaccinations in PDF format during the first week of class.

• MMR (2)
• Varicella (2)
• Covid-19 (Fully vaccinated)
• Flu Shot (for the flu season in which you begin class) or completion of NAH Declination Waiver
• Tdap (received within the past 10 years)
• TB Skin Test (Good for one year & cannot expire before the end of the clinical rotation, typically April 30th)

Please provide the name and contact details for three professional references who have known you for at least one year. References should be familiar with your history of reliability and responsibility at work. Do not list relatives.
