Advance review of these instructions and necessary materials for your application is highly recommended, and it is the applicant’s responsibility to utilize these most up to date guidelines for the current application cycle. The Nursing Department will only look at applications after the posted deadline to ensure fairness in the application process. If you recognize you made any errors on your application after it has been submitted, you are welcome to resubmit your application with adjustments and all attachments before the deadline. If anything on your application (including attachments) is not provided, does not show your name, or is inaccessible to us, then we cannot consider your application complete, and it will not be reviewed.

 

The Nursing Department maintains updated guidelines for each application cycle, and these are the requirements for the application:

• Completion of this Nursing Program Application
• Pre-requisites of ENG 101, ENG 102, MAT 140 (or higher), BIO 201, BIO 202, and CHM 130
• Grade of “C” or higher in all pre-requisite courses
• Pre-requisite grade point average of 3.0 on a four-point scale or higher
• A minimum 50th percentile rank or higher on NLN NEX test taken within 12 months of application deadline with no more than 2 attempts in a year.
• AZ CNA or LNA license or completion of a Nursing Assistant Program in the semester immediately preceding the application deadline.
• The following necessary attachments:
1. Unofficial Transcripts
2. NLN NEX Results
3. CNA or LNA License verification from the Arizona Board of Nursing or Certificate of Completion from a registered Nursing Assistant Program in the semester immediately preceding the application deadline.

 

When uploading attachments, please ensure the following:

• Legal name is displayed that is consistent with the name that you are using on the application.
• If a different name is displayed on any of your documents, then you will need to provide proof of a namechange, either through a marriage certificate or court order.
• Please do not use screenshots as your attachments, as it is often difficult to ensure that your name and the rest ofthe required information show simultaneously.
• Please ensure that the attachment is not protected or that it is not in a version that is not universally accessible(the preferred version is a pdf).

 

Once completed, you will receive an automated confirmation email saying that we received your application

Please list the specific course that satisfies the pre-requisite requirements using the example below.

If you are using a pre-requisite that is not one of the course numbers listed below, then you must use the Arizona Course Equivalency Guide to determine if your course will meet the requirement. This is the guide that the nursing department will be using when evaluating your application.

Transcripts are required to be attached alongside this application showing all pre-requisite courses completed with a final grade, from every institution that granted you the credits.

• If your pre-requisite is a transfer course, you must attach the transcript from the institution that granted you thecredits (the institution that you took that specific class with).
• If you are a CCC student, you will still need to upload your unofficial CCC transcript.
• Please do not send official transcripts to us, as they will not be attached to your application. If your school onlyprovides official transcripts, send them to yourself, open them, and scan them as a document attachment to yourapplication.

 

We require the AZ CNA or LNA License for our Nursing AAS degree.

If you have taken the Nursing Assistant course the semester immediately prior to the application deadline (i.e., Fall semester for March deadline) then you may use your certificate of completion in lieu of the CNA or LNA license for this application. However, this is only applicable for those students who have completed a registered nursing assistant program the semester immediately preceding the application deadline. Otherwise, you will need to provide proof of our current and valid Arizona CNA or LNA license with the application.

For Proof of your valid CNA or LNA License, Verification from the Board of Nursing is sufficient that lists your name, license number, indication that it is “active,” and expiration date.

• Your CNA or LNA license must be active at the time the application is due.
• Please download your report instead of taking a screen shot.
  
  

For proof of your successfully completed nursing assistant course, please attach your Certificate of Completion that was granted to you at the end of the program

• The Nursing Assistant course must be registered with the Arizona Board of Nursing.
• You must be able to provide proof of a valid CNA or LNA license by the date listed on the acceptance packet toenroll our nursing program. Therefore, we strongly advise you to initiate the process for obtaining your LNAlicense through AZBN as soon as possible, as response times from AZBN can vary.
  
  

We cannot accept an NLN NEX score below the threshold of the 50th percentile rank or outside the eligible testing dates of the 12 months preceding the application deadline.

Please download your official NLN NEX results and upload it as an attachment to this application, rather than taking screenshots of your results.

*The previous application will be reviewed for this qualification.

If you would like the additional application points for having 40 hours of direct patient care in the past six months, this can be from one of the following: employment, volunteer, satisfaction of clinical requirement for CNA program. For verification, we prefer that you use our Healthcare Experience Verification Form, which can be found under Forms on the CCC Nursing Program Webpage. If a verification is not provided, then we cannot appoint the Healthcare points to your application.

I certify that all the information provided by me is true, complete, and accurate. I acknowledge that I have read and understand the admission requirements for the CCC Nursing Program and have only used the information published on the CCC Nursing Information Webpage. Falsification or omission of pertinent data on the application will forfeit admission to the Associate of Applied Science in Nursing degree. I acknowledge that admission to the program is conditional until I have officially received an acceptance letter and fulfilled all the health requirements set forth by the Nursing Program. Failure to complete this application will result in invalidation of the application, and it will not be reviewed for admission.
